Vision and Hearing Notice

Vision and Hearing screenings are required by Virginia Code 22.1-273 for all students new to Prince William County Public Schools and for students in grades K, 3, 7, and 10. These screenings will be conducted by the school nurses within 60 days of starting school. When appropriate, referrals for follow-up care with a private health care provider will be sent home. School nurses may also check vision and hearing when requested by educators, parents, or students. If you have updated information from your health care provider, please call the school nurse and send a copy of the report. Parents may also exclude students from a specific screening with a written notification to the school nurse.

CLINIC PASS

Students who need to see the nurse for any reason is given a clinic pass from the teacher. This clinic pass will be returned with the student after the nurse/clinic attendant has added their notes. The clinic pass will go home with the student so parents can be informed of any visits their child had to the clinic. The pass will be a yellow carbon copy sheet. Phone calls will still be made when deemed medically necessary.

MEDICATIONS AT SCHOOL

  • All medicine must be in its original container, absolutely UNOPENED, with the student's name."
  • All medicine (over the counter or prescription) administered at school, must have a medication authorization form on file in the clinic.
  • Prescription medicine must have a physician's signature on the form before medicine can be administered.
  • Absolutely NO medication is to be sent to school with the student. If a student is known to be carrying any type of medication, it will be confiscated by and kept with Nurse Ana until a parent or guardian retrieves it from the clinic. Therefore, parents please bring to the clinic any and all medication with its corresponding Medication Authorization Form signed by a physician.
  • Cough drops are no longer considered medication and can be carried by students without medication form. Cough drops need to be in their original package and not shared with other classmates.
  • Students with allergies and/or asthma should have an asthma and/or allergy plan on file as well.

 


Important News & Resources

If your student has a medical condition (asthma, severe food allergies, diabetes, migraines...), the clinic MUST have a Health Treatment Plan in order to treat your student with ANY medications.

For parents of children with ASTHMA, please make sure that the clinic has an inhaler and an Asthma Action Plan signed by you and their medical provider.

For SEVERE ALLERGIES, the clinic must have an Allergy Action Plan signed by you and a medical provider. This must be given to the school before any Epi-pens or other medications are delivered.

If you would like MEDICATION to be kept in the clinic (pain, allergies, cough/congestion), you must provide a signed Medication Authorization form (only 1 medicine per page).

*ALL of the above forms MUST come with an AUTHORIZATION TO IMPLEMENT HEALTH TREATMENT PLAN. *


Attention PWCS Parents:

All preschool, kindergarten, and new to the county students are required to submit a Virginia School Entrance Health (Physical) Form and immunization record, completed by a licensed provider, prior to their first day of school. School entrance physicals must be completed within one year of entry. Students who were previously enrolled in a PWCS preschool program, through Head Start or Child Find, are also required to submit an updated physical that falls within the dates above. This is in accordance with the Virginia Department of Education school entrance requirements and is a requirement for both in-person and virtual schooling within the county. These records must be provided to the school nurse prior to beginning school. Commonwealth of Virginia Minimum Immunization Requirements

**Effective July 1, 2021** A minimum of 2 doses of Hepatitis A vaccine are required for school entry. The first dose should be administered at age 12 months or older.
